Following Admission for Rehabilitation
Within one working day of admission, the Rehabilitation Team meets with the patient and their family to discuss their rehabilitation programme.
All therapy assessments are completed; the team will agree achievable personal goals with the patient and their family. We will adapt the rehabilitation programme as time goes on to ensure optimum progress is maintained.
All members of our team are sensitive to a patient's psychological, cultural, spiritual and social needs and every programme is designed with these factors in mind.
We are particularly careful to include advice and education for the patient and family members on how to manage and make the most of the patient's potential after he or she leaves our hospital.
A goal-orientated patient-centred approach to treatment is adopted with goals reviewed every two weeks.
